Instructions for completing speaker form:
Fill in each section as completely as possible. Some examples of acceptable verbiage from the continuing education office is listed below.
Speaker Documents:
Please only create one objective using verbiage from the knowledge section. (Ex: Cite the essential components of antenatal discussion) . The remaining objectives should come from any of the other sections. (Ex: Describe the following congenital abdominal wall anomalies: umbilical hernia, omphalocele, gastroschisis, & pentalogy of cantrell.)
For the scholarly references, please use a minimum of three. The five test questions must be multiple choice. Please provide three to four answers as choices.

The speaker form should be returned to the IDHI program manager at least 10 business days prior to your scheduled lecture date. If this form is not completed and returned by the deadline, you risk having to reschedule your lecture.
Writing Learning Objectives 101 As learning objectives should be measurable, they should begin with a verb that can be measured, e.g. understand, know is NOT on the list because one’s understanding cannot be readily measured. The following list is designed to help you create your objectives. Try to be specific and vary the verbs.
